Tennis elbow or lateral epicondylopathy, is a common overuse injury that can make daily tasks frustrating and keep you off your game. Despite the name, it’s not just tennis players who develop it. Anyone who does repetitive arm movements, from painting and typing to playing instruments or gardening can be affected.

What Is Tennis Elbow?

Tennis elbow is pain and tenderness on the outside of the elbow, at the bony point called the lateral epicondyle. The problem usually starts where the extensor carpi radialis brevis (ECRB) tendon attaches. It occurs when the tendon is repeatedly overloaded, especially during activities that involve gripping and wrist extension.

In simple terms: the muscle is contracting while being stretched at the same time. Imagine pulling on a rubber band in two directions at once. Over time, this places stress on the tendon. While racquet sports are a common trigger, we see it just as often in tradespeople, gardeners, and weekend DIY-ers.

Signs and Symptoms

  • Pain or tenderness on the outer elbow, especially when gripping or twisting (examples: like turning a key; turning doorknob; pouring the kettle, using a screwdriver, shaking hands)
  • Weak grip strength, making it harder to lift or hold objects
  • Stiffness, particularly in the morning or after rest

Treatment and Physiotherapy

The good news: most cases improve with conservative management over time, often within 6 to 12 months.

Treatment usually includes:

  • Rest, ice, and support: to manage pain in the short term.
  • Physiotherapy exercises: targeted stretching and strengthening for the forearm, wrist, and even shoulder, to restore function and resilience.
  • Manual therapy: massage and joint mobilisation to reduce discomfort
  • Education and advice: on ergonomics, technique, and activity modification, whether that’s adjusting your tennis swing or using power tools more safely.
